Parental Opt-Out form for FERPA notification

Under FERPA, Liberty Central School District is generally required to obtain written consent from a parent or guardian before disclosing personally identifiable information from a child’s education records. However, FERPA allows for the disclosure of “directory information” without obtaining written consent unless a parent or guardian has informed us otherwise in accordance with our procedures.

Directory information is data that is not considered harmful or invasive if released and is primarily used for specific school publications and related purposes. This includes but is not limited to the following:

  •  A playbill, showing a student’s role in a drama production
  •  The annual yearbook
  • Honor roll or other recognition lists
  • Graduation programs
  • Sports activity sheets, such as for wrestling, showing weight and height of team members

Additionally, outside organizations, such as companies that manufacture class rings or publish yearbooks, may also receive directory information without prior written consent.

We want to make sure that preferences regarding the disclosure of directory information are respected. Therefore, those who do not want Liberty Central School District to disclose any or all of the types of information designated as directory information from their child’s education records without your prior written consent, please notify us in writing by Sept. 29 of the current school year by emailing dataprivacy@libertyk12.org or mailing the request to Stacy Feasel, Privacy Officer, 115 Buckley Sr., Liberty, NY 12754.

The following information has been designated as directory information by Liberty Central School District:

  • Student’s name
  • Address
  • Telephone listing
  • Electronic mail address
  • Photograph
  • Date and place of birth
  • Major field of study
  • Dates of attendance
  • Grade level
  • Participation in officially recognized activities and sports
  • Weight and height of members of athletic teams
  • Degrees, honors, and awards received
  • The most recent educational agency or institution attended
  • Student ID number, user ID, or other unique personal identifier used to communicate in electronic systems but only if the identifier cannot be used to gain access to education records except when used in conjunction with one or more factors that authenticate the user’s identity, such as a PIN, password or other factor known or possessed only by the authorized user
  • A student ID number or other unique personal identifier that is displayed on a student ID badge, but only if the identifier cannot be used to gain access to education records except when used in conjunction with one or more factors that authenticate the user’s identity, such as a PIN, password or other factor known or possessed only by the authorized user.
